Respiratory Simulators Used in China's National Medical Exams

The acceptance of simulation based teaching and training is growing quickly in China. One example is the utilization of lung simulators to design new respiratory care devices and another is the utilization of simulators to explore the effect of different intensive care ventilation strategies.

Most recently, using simulation based principles, the China National Medical Examination Center (NMEC) has initiated an examination program for clinical specialists.

Four cities in China (Chengdu, Beijing, Shanghai, and Guangzhou) were chosen by NMEC in September of 2019 to hold examinations using medical simulation tools for clinical practitioners under multiple disciplines.

Mechanical ventilation is one of the disciplines, and for this the TestChest from Organis GmbH, Switzerland, was utilized as the respiratory simulator to simulate different respiratory diseases, alarms and emergency situations. Case development was performed together with Trinity Trading Co. Ltd.

The topics of examination with TestChest included the aspects of mechanical ventilation below:

  • Lung Protective Ventilation Strategies
  • Modes of Ventilation and Alarm Setting
  • Ventilation Modes and Waveform Analysis
  • Respiratory Compliance
  • Lung Recruitment Maneuvers, PEEP Titration
  • Transpulmonary Pressure
  • Lung Mechanics
  • Lung Recruitment Maneuvers and AutoPeep

The following teaching institutions were involved:

  • Sir RUN RUN SHAW Hospital of Zhejiang University School of Medical
  • China-Japan Friendship Hospital, Beijing
  • The Children’s Hospital of Chongqing Medical University (CHCMU)
  • The General Hospital of People’s Liberation Army (301 Hospital), Beijing
  • Beijing Chaoyang Hospital of Capital Medical University , Beijing
  • The First Affiliated Hospital of GuangZhou Medical University, Guangzhou

The examinations were held on different days and were completed successfully.

The China National Medical Examination Center is near the Kunyu River in Haidian District, Beijing and was founded in 1985. NMEC serves as an examination organization in the health field as it is an affiliated unit to the National Health Commission, and is one of the earliest professional examination institutions in China.

NMEC licenses qualified medical practitioners and is the main publisher of National Medical Licensing Examination (NMLE), which is enforced by Law of the People’s Republic of China on Medical Practitioners as the mandatory qualification examination for medical practice applicants.

NMLE is divided into a general knowledge test and a clinical skill test; the candidates who pass the NMLE are awarded the qualifications of medical practitioners. NMEC can contribute to the publication of high-quality medical examinations and is willing to cooperate with international counterparts.
