Approximately half of male prisoners in England and Wales will meet diagnostic criteria for ASPD. The majority of such men are not true psychopaths (ASPD-P). They are characterized by emotional instability, impulsivity and high levels of mood and anxiety disorders. They typically use aggression in a reactive way in response to a perceived threat or sense of frustration.

Antisocial personality disorder is defined as "a pervasive pattern of disregard for, and violation of, the rights of others that begins in childhood or early adolescence and continues into adulthood," according to the fourth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) of the American Psychiatric Association.

Researchers have found that inability to recognize others' insincerity, lies and sarcasm could be a warning sign of a form of dementia. They found that people with frontotemporal dementia - a type of dementia that affects the frontal and temporal lobes of the brain - often lose the ability to detect sarcasm and lies, while people who grow older normally don't experience this.
