    The lack of open competition across states is a major factor, but I would ask you to consider the distinct possibility that someone (federal and state politicians) are getting paid off by the insurance companies and lobbies to insure competition is restricted.  It is past time for term-limits and voting all of these criminals out of office. They look out only for themselves.
