LDM expands professional and patient communication network with Kmart

Through this partnership with LDM, Kmart will now offer PharmacistCare™ and CarePoints® messaging solutions to pharmacists and patients which help improve patient medication adherence and overall health outcomes.

PharmacistCare offers a unique way for pharmacy staff to stay updated on all new developments in the industry and provide needed education that will improve the pharmacist-patient dialogue. It is an automated real-time intervention solution that delivers messages directly to the pharmacist in their workflow. Such communications include updates on new products, indications, strengths or dosing, formulary updates, coverage updates, and awareness of patient programs or savings opportunities.

CarePoints personalized messages are printed in real-time and distributed by the pharmacy staff to the patient, along with the patient's prescription. The targeting message is based on current therapy and medication history that includes information about the patient's prescription as well as information about the condition being treated. CarePoints can include valuable educational, adherence and financial incentives that positively change a patient's behavior, while preserving patient privacy. In addition, pharmacies, like Kmart, can use CarePoints to communicate to patients about in-house health services, promotions, and private-label products. CarePoints is integrated within the pharmacy system and is delivered in workflow for maximum impact on patient behavior.
