PointRight® Pro 30™ rehospitalization, included in the PointRight Care Transitions family, is the industry-standard case-mix adjusted measure adopted by the American Health Care Association (AHCA) and incorporated into its Trend Tracker product. AHCA also recently submitted the measure to the National Quality Forum (NQF). The measure has also been validated by Brown University.
