AultCare reports positive results from Cardiocom program

NewsGuard 100/100 Score

AultCare has been using patient telemonitoring for six years and has, in partnership with Cardiocom, released a white paper entitled, "AultCare Sees Positive Results from Cardiocom Program."

The paper describes the structure of the coordinated program between health plan, hospital, physician and patient. Improved patient self-management has resulted in measurable reductions in emergency room and inpatient utilization from participating patients. With health reform incentives to hospitals to reduce readmission rates, and initiatives to improve care coordination for chronic conditions, this data reveals timely information for patients and health systems.

The white paper discusses:

  • How telemonitoring works
  • Results achieved
  • Lessons learned
