Saint Francis HealthCare Partners and Phytel partner to achieve NCQA recognition

Saint Francis HealthCare Partners announced today that it has selected Phytel to help achieve NCQA recognition as a patient-centered medical home and will be rolling out Phytel's unique care coordination and population health management solution, Proactive Patient Outreach, in November. As part of the continual effort for quality improvement, Phytel is sponsoring the 2010 Annual Conference presented by Saint Francis HealthCare Partners and the Connecticut Institute for Primary Care Innovation, taking place on October 26, 2010 at the Hartford Marriott in Farmington, Conn.

The conference will educate healthcare professionals about the patient-centered medical home, the impact of information technology and health information exchange on building the medical home, and the medical home's role in accountable care organizations (ACOs) and EHR meaningful use. Keynote speakers include leading experts, such as Paul Grundy, M.D., M.P.H., FACPM, president of the Patient Centered Primary Care Collaborative; Pauline Chen, M.D., author and columnist for the New York Times; Gregory Makoul, Ph.D., chief academic officer/senior vice president for Innovation and Quality Integration at Saint Francis Hospital and Medical Center; and Johann Chanin, director of product development for the National Committee for Quality Assurance (NCQA).

Phytel's Proactive Patient Outreach system creates an electronic registry of patients who require preventive and chronic care, based on nationally validated, evidence-based protocols and data from the organization's electronic health record and practice management system. The technology identifies gaps in care, triggers automated messaging about recommended visits, tests or procedures, and tracks the results, including quality outcomes data for quality reporting.
