eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced American Health Network (AHN) is live using eClinicalWorks Care Coordination Medical Record® (CCMR) for population health management to manage three Accountable Care Organizations (ACOs) - American Health Network of Ohio Care Organization, Franciscan AHN ACO and Indiana Care Organization. These ACOs have a total of 51,663 Medicare members and 320 providers across Ohio and Indiana. As part of this relationship, eClinicalWorks provided the technology and resources to assist all three ACOs with their successful submissions for Group Practice Reporting Option (GPRO) in 2013, a requirement for Medicare ACOs.
"At American Health Network of Ohio Care Organization, we strive to improve the health of our patients and demonstrate our core values of teamwork, accountability, acknowledgement and empowerment," said Mike Kirschner, CFO at American Health Network of Ohio Care Organization. "Working with different ACOs that have disparate EHR systems required us to find a vendor-neutral, complete solution. We selected eClinicalWorks CCMR as one of the most comprehensive population health management solutions available and it has been key in our successful GPRO submissions along with reducing costs and meeting additional care goals."
The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. The AHN ACOs participate in the Medicare Shared Savings Program, which has been created by CMS for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR is a Web-based, population health management platform for supporting ACOs along with other shared-savings models and will aid AHN, and its partner ACOs, in meeting its quality measures and performance metrics.
"AHN is taking tremendous strides to improve health in both Indiana and Ohio," said Girish Navani, CEO and co-founder of eClinicalWorks. "eClinicalWorks has taken its extensive knowledge of clinical workflow and created a top-rated, comprehensive population health management platform. With eClinicalWorks CCMR, organizations can measure, analyze, improve clinical quality outcomes, and leverage evidence-based care from a single solution. We look forward to continuing our work with American Health Network."
eClinicalWorks CCMR will be providing American Health Network of Ohio Care Organization with visibility into a patient's care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
