Axial Exchange launches new Spanish-language version of mobile application

Axial Exchange, Inc., a pioneer in using mobile applications to deepen the patient's role in improving outcomes, launched a new Spanish-language version of its mobile application, making the company's innovative patient engagement tools available to a greater population.

With Axial Exchange's applications, patients are able to access personal health information in an easy-to-use interface, helping them learn about their medical condition(s), set medication reminders and alerts, and track progress against designated care plans. Among other benefits, the application also provides interactive health trackers that monitor weight, glucose, blood pressure, medication adherence, and other metrics that are essential to health and wellness. The self tracking features have been translated into Spanish, enabling those for whom Spanish is a primary language to be more engaged in their care by tracking, managing and analyzing their health and wellness on mobile devices.

"As the Affordable Care Act broadens the availability of healthcare to millions of uninsured Americans, we wanted to expand the reach of our patient engagement tool so that it will be available and useful to a wider population," said Joanne Rohde, CEO of Axial Exchange. "Axial's patient engagement application enables enhanced patient and family involvement in care and ultimately decreases costs of healthcare for individuals, families and eventually, an entire population as engagement increases."

Axial's solutions are used by more than 70 provider organizations across the U.S. and focus on improving patient engagement and clinical outcomes. Axial Exchange won first prize in the U.S. Department of Health and Human Services' Partnership for Patients initiative on the Ensuring Safe Transitions from Hospital to Home Challenge run by the Office of the National Coordinator (ONC).
