To help employee members take a more proactive role in managing their own health programs, Avivia Health From Kaiser Permanente today announced the introduction of Your Health Center, a new portal based on Web 2.0 technology that provides user members with powerful digital health coaching and interactive tools. Avivia Health is the only national provider of Wellness and Condition Management Solutions based on 60 years of evidence-based practices by its parent organization, Kaiser Permanente.

“We're providing a way to serve information intelligently to users based upon a continually evolving series of data points and detailed understanding of their needs”

Your Health Center is a dynamic web platform that intelligently connects users with personally tailored content, self-service tools and provides access to high quality clinical information and interactive tools - all of which are designed to help users select and maintain the right wellness programs or manage chronic diseases.

"This is a major step forward in wellness and condition management," said Barry Cambron, Vice President of Marketing and Business Development for Avivia Health From Kaiser Permanente. "We're driving important behavior change here through powerful, state of the art technology that gives our user members more relevant information more quickly than possible before. Most significant, we've been gratified by market acceptance of the portal since we just signed on a global technology company as our newest customer. The web portal technology was a major factor in their decision to select Avivia Health as their population health management solution."

By developing unique user profiles based on the analysis of medical claims files, Health Assessment results and web usage statistics, the portal enables the execution of system-driven, individually targeted outreach campaigns. The portal promotes member self-service through "intelligent" modular web-based programs and tailoring of clinical programs based on specific member data and self-entered information. Employee members can also use the portal to get important clinical information such as symptom checkers, information about urgent conditions, medicines and clinical conditions, diaries and self-care tools, Digital Health Coaching and links to additional communities and 24-hour fitness resources.
