Outcomes detects over one million drug therapy problems through Targeted Intervention Program

Outcomes Pharmaceutical Health Care® has announced it has detected over one million drug therapy problems through its Targeted Intervention Program (TIP®).

Outcomes, a national administrator of Medication Therapy Management (MTM) services, accepts prescription and other medical data from health plans and utilizes a company-developed clinical rules engine to analyze the data. Identified drug therapy complications are then communicated in a patient- and intervention-specific format, known as TIPs, to the Outcomes network of specially-trained Personal Pharmacists™. Each TIP provides guidance to pharmacists on working with patients and doctors to fix medication complications.

"The Targeted Intervention Program arms pharmacists with actionable clinical information that otherwise may not be easily accessed in community pharmacies."

TIP protocols focus on non-compliance, adherence with current disease state guidelines, plan-specific formulary management, use of potentially inappropriate medications for the elderly and other therapeutic areas. The protocol library includes a set of protocols that target nine measures from the Healthcare Effectiveness Data and Information Set (HEDIS).

The company currently works with over 30 US health plans that have adopted the Outcomes unified MTM platform which connects such plans with 33,000 chain, independent, consultant and health-system pharmacy providers across the country to improve patient care.
