PES' ESRD Prevention Program effectively identifies patients with early stage renal disease

Patient Engagement Systems® (PES), a nationally recognized leader in developing technology to identify, and monitor individuals with chronic kidney disease (CKD), today introduced the Patient Engagement Systems' ESRD Prevention Program℠ which uses proprietary technology including automated physician-patient engagement, analytics, and decision support tools to identify patients with undiagnosed chronic kidney disease and tools to monitor them in order to prevent or delay the progression to ESRD.

Patient Engagement Systems' ESRD Prevention Program provides up to 80% greater identification of patients with early stage renal disease. According to the American Society of Nephrology, if detected early in its progression, kidney disease can be slowed and the transition to dialysis delayed.

Physician Medical Groups who have piloted the Patient Engagement Systems' ESRD Prevention Program for more than one year have demonstrated better outcomes and lower costs through fewer inpatient admissions and ER visits.

The National Kidney Foundation reports that treating patients with ESRD costs an average of $70,000 per year.

Patient Engagement Systems' ESRD Prevention Program includes three modules:

  • Detection and Surveillance
    This automated module identifies patients with CKD by reviewing current and historical laboratory data on a daily basis to identify and stage patients with kidney disease.
  • Patient Engagement & Communications
    This module is the engine that produces automated communications from the provider to the patient with the goal of getting the patient back into primary care at appropriate times. All communications are timely and personalized.
  • Analytics and Clinical Decision Support
    Clinical decision support for the primary care physician is provided by this third module. Flow sheets including clinical decision support enable the physician to more easily manage these patients. Also included are the tools clinic managers need to better manage physicians and their chronic disease patients. These include clinical work lists, and population management reports.
