Republicans on both sides of the Capitol are investigating technical problems with the health law's online health insurance marketplaces -- with one senator going so far as to call for the resignation of Health and Human Services Secretary Kathleen Sebelius. Others bemoan the lost opportunity to publicize the balky rollout because public attention was focused instead on the government shutdown.

The Associated Press reports that some insiders predicted the problems well before the website launch, while The Washington Post notes that paper applications are increasingly popular because of the problems.

WHO: Iron deficiency causes loss of 48,225 disability-adjusted life-years worldwide

Data supporting the need for better recognition of iron deficiency will be presented this week at the United European Gastroenterology Week Congress in Berlin. Iron deficiency is frequently under-diagnosed, resulting in increased hospitalisation, reduced quality of life and higher death rates in patients with comorbid conditions. The WHO has estimated that iron deficiency causes the loss of 48,225 disability-adjusted life-years worldwide, more than is seen with lung cancer.

A new gene transfer strategy that introduces modified, immune-stimulating human stem cells is both feasible and effective for achieving persistent immunotherapy to treat leukemias and lymophomas, according to a study published in Human Gene Therapy, a peer-reviewed journal from Mary Ann Liebert, Inc., publishers.

Politically, the biggest challenges facing the ACA are behind us. By my count, the law has already survived three near-death experiences: the Democrats' loss of a filibuster-proof Senate majority with the election of Scott Brown in Jan. 2010, the Supreme Court's June 2012 decision upholding (with one exception) the law's constitutionality, and President Obama's re-election last Nov. Much of what we see going on in Washington now is the death throes of the opposition, especially now that Americans are beginning to sign up for coverage (John McDonough, 10/9).

We're awash these days in metaphors as overworked as our political debate is overwrought, and it's impossible not to wonder how much one contributes to the other. When nuance and perspective exit the language, do they exit the conversation as well? When you speak in ludicrous extremes, do you think that way, too? Obamacare has proved to be not just ideologically divisive but linguistically fertile. There's seemingly no event or passage in American history to which it can't be compared (Frank Bruni, 10/7).
