Health law's insurance exchanges studied, demonstrated

According to data from Connecticut insurers, the majority of health plans sold to individuals and small businesses provide less generous coverage -- most likely, at a lower cost -- than plans sold on the exchange will be required to provide. The analysis suggests that people buying insurance through the exchange could get plans that offer more coverage with lower out-of-pocket costs, but they would likely have to pay more for them.

Republicans advance plan to charge wealthy beneficiaries more for Medicare to offset cost of payroll tax extension

The proposal would require people who earn more than $1 million to pay the full cost of Medicare. Meanwhile, House Majority Leader Eric Cantor, R-Va., is floating a "trigger bargain" that would delay for one year the automatic spending cuts scheduled to begin in 2013 and also could include a payroll tax extension and a fix for physician's Medicare payment rate.

Insurance commissioners back brokers on health law standard

After more than a year of debate, state regulators ... approved a resolution calling for changes to the healthcare reform law's medical loss ratio provision. The National Association of Insurance Commissioners (NAIC) passed the measure 26-20, over strenuous objections from commissioners who said the process and the resolution itself would undermine the NAIC's reputation.

Health industry raising alarms about cuts to Medicare after super committee failure

Automatic Medicare cuts triggered by the congressional super committee's failure would hit hospitals harder than any other health industry, according to an analysis from Avalere Health. The report puts hard numbers to across-the-board cuts that stakeholders have mostly viewed in broad, general terms. But Avalere chief executive Dan Mendelson cautioned that the figures also assume the automatic cuts will actually happen - an assumption he's not ready to make.

States weigh health law's expected positive and negative impacts

Connecticut now has more health benefit mandates than nearly all other states, a consultant told a state board Thursday. And when federal health reform rolls out in just over two years, that could leave the state, not the insurers, with a bill to pay. ... Although it's not yet clear what benefits federal government will require, as many as three-quarters of the benefits that Connecticut mandates could exceed what the federal government requires.

CMS delays Florida MLR decision; Conn. exchange board under fire

Centers for Medicare and Medicaid Services officials have given themselves 30 more days to decide on Florida's request to slow the phase-in of a requirement that insurers spend a minimum of 80 percent of each premium dollar on expenses related to improving the quality of health care.

State highlights: Wash. Gov. announces more budget cuts
