A large proportion of teenagers regularly and frequently take some form of medication without receiving targeted information about the risks and benefits, according to a review of current research, to be presented at the annual congress of the International Pharmaceutical Federation.

So the current exercise is about rooting out $1.5 trillion in "savings," which will mean a hodgepodge of changes at the margin, not a rewrite of either entitlements or taxes. If bigger change were possible, Speaker John Boehner and President Obama would have agreed to it.

All politicians—but these days especially Democrats—like to pretend they'll pay providers somewhat less in the future, despite knowing that is unlikely to happen in practice. Medicare's prospective payments are low enough that further reductions may jeopardize access to care and in many cases threaten the viability of hospitals and physician practices.

The agreement sets up a powerful mechanism for agreement on tax reforms to strengthen growth, and entitlement reforms to strengthen programs such as Medicare. A congressional committee with fast-track authority will have a Nov. 23 deadline to recommend a balanced package of long-term reforms to produce $1.5 trillion in additional deficit reduction.
