In all the recent discussions about health care reform, what had heretofore played out only beyond earshot of the exam room suddenly was very public: the tangled, uneasy and often antagonistic relationship between practicing doctors and the insurance companies who pay for the services they deliver (Dr. Pauline Chen, 4/29).

Health reform politics still swirling; SEIU close to choosing new leader

CongressDaily: "When House Minority Leader Boehner ranks wholesale repeal of healthcare reform as Republicans' No. 1 priority, that thinking might be more wishful than pragmatic." If the GOP scores significant gains in the upcoming midterm and 2012 elections, they could draw Democrats back to the table to renegotiate certain parts of the law.

Health law keeps 200,000 in state insurance plans instead of new high-risk pools

USA Today: About 200,000 people already enrolled in 35 state high-risk insurance pools "will not be allowed to enroll this summer in a new lower cost federal program for people like them because they already buy pricey state-run plans." In order to be eligible, "a person can't have had health coverage for six months.

UnitedHealthcare ends use of rescissions ahead of implementation date

UnitedHealthcare, a UnitedHealth Group company, has ended its limited use of rescissions effective immediately, except in cases of fraud or intentional misrepresentation of material fact. This change in policy is well ahead of the September 23, 2010 implementation date as required in the recently passed health reform legislation.

Experts disagree on health law's effects on jobs

"There is profound disagreement about healthcare reform's overall impact on jobs. … While the net effect on jobs remains uncertain, it is clear that certain professions will do better than others." Among the specific job categories likely do well are insurance agents, who "may get a boost since approximately 32 million uninsured Americans will be required to buy health insurance starting in 2014.

More taxes and drab packages for cigarettes to deter the young from smoking

From 1st July 2012 companies making cigarettes will be allowed only to have a brand name and a graphic health warning on their cigarette packs. There has been a huge debate over this issue. Trade experts believe that this move could mean that the Federal Government may have to cough up more than $3 billion a year to cigarette companies. Tim Wilson from the Institute of Public Affairs also believes that this would cost taxpayers $3.4 billion a year in compensation to tobacco companies. However Australia will be the first country to introduce such a measure.

State Round-up: Federal prosecutors investigate bid rigging at New York hospitals; Montana prepares for health reform implementation; and Florida spares Medicaid from budget cuts

The Wall Street Journal: "Federal prosecutors are investigating allegations that bid rigging and fraud at Mount Sinai Medical Center and New York-Presbyterian Hospital resulted in the hospitals awarding contracts worth tens of millions of dollars to outside contractors. Purchasing officials at the hospitals ... are alleged to have gotten more than a million dollars in payments from companies that were then given lucrative contracts to perform work such as re-insulating pipes and removing asbestos, according to documents filed in the Southern District of New York.

Moody's report: States will feel pinch of reform's Medicaid, health care expansions

"The Medicaid expansion under health reform is expected to strain state budgets already distressed by the recession, despite delays for new eligibility rules and federal aid, Moody's Investors Service said in a newly released report. States that see the largest gains in Medicaid enrollment under health reform will likely see the greatest financial burden, Moody's said."

Fine print complicates effort to cover young adults

Some parents may have to wait to get children as old as age 26 to be allowed to stay on their health plans because of the health law's fine print, The Associated Press reports. "As a consequence, some families may have to wait until 2011 to get their kids covered, particularly if the parents are working for a large employer, benefits experts and government officials say.

As health reform's specifics continue to emerge, so do questions and answers from stakeholders

Determining how the law will be applied will be the next big battle in health reform and lobbyists are gearing up to influence implementation. "Congress gave sweeping power to federal agencies, especially (HHS), to fill in gaps lawmakers left in the 906-page legislation — an effort that will take years. The law refers more than 1,000 times to Cabinet secretaries who will make decisions on how to carry out the law. For example, the law requires insurance companies to spend 80% of premiums on medical claims, as opposed to administrative costs, by 2011. But it directs the health department to decide whether gray-area expenses, such as health-and-wellness programs offered by insurers, count as care or overhead."

States uneasy about impact of Medicaid expansion

The expansion of insurance coverage under the health reform law is due largely to an expansion of the government-funded Medicaid program, which is causing some doctors to worry about the specifics of implementation, according to the New York Times. "Absorbing that many people into the system will not be easy. The program is administered and partly financed by the states, which are now racing to figure out how to carry out the necessary changes and simplify enrollment even as they struggle to cope with severe budget cuts and staff shortages."
