    4D CT scanning is not used to diagnose primary hyperparathyroidism.  The diagnosis is strictly biochemical: what are calcium levels in comparison to PTH levels.  If the PTH is not suppressed with a high-normal or frankly elevated calcium, then you have the diagnosis.  4D CT scanning is very expensive (in comparison to US) with exposure to nephrotoxic contrast and high doses of radiation.  To suggest that 4D CT is the test of choice to "diagnose" primary hyperparathyroidism is inaccurate and misleading.
