Inflammatory markers as well as atherosclerosis are not the same thing as a heart attack. Oral estrogens increase thrombosis risk, which may explain the higher risks of stroke and MI from birth control pills in YOUNGER women - who presumably have "clean" arteries. Moreover, there is suggestion that (more physiologic) transdermal estradiol, based on the parallel KEEPS trial, not only does not have oral HT's artery scrubbing benefits, it may exacerbate atherosclerosis risk in newly menopausal women via a greater adverse effect of paracardial fat deposition on CAC progression.
