State highlights: Mass. health care system eyes new expansion; some health program cuts not addressed in Calif. gov.'s budget

Facing challenges in their home state, top executives of Boston-based Partners HealthCare System told a national audience of investors Monday that they will create a bold "new medical model" by integrating hospitals and their medical services with insurance products and by drawing patients from across the country. Speaking at the J.P. Morgan Healthcare Conference, Partners' chief executive, Gary L. Gottlieb, said his organization, which owns Harvard-affiliated Massachusetts General and Brigham and Women's hospitals in Boston, plans to improve medical care and lower costs by further expanding its network of community hospitals and primary care physicians in Eastern Massachusetts (Weisman, 1/14).

More than a dozen states have passed legislation expanding the role of nurse practitioners (NPs) so they can help fill the shortage of primary care providers in the U.S. But a new study from Columbia University School of Nursing, published in the Journal of Professional Nursing, suggests that the organization and culture in health care settings often impedes the full utilization of NPs so that state laws expanding scope of practice may not be sufficient to relieve the nation's primary care crisis.

Quality of life considerations are key when patients consider bariatric surgery

While weight loss surgery offers one of the best opportunities to improve health and reduce obesity related illnesses, the nearly 100,000 Americans who undergo bariatric surgery each year represent only a small fraction of people who are medically eligible for the procedure. Among those who have surgery, Caucasian Americans are twice as likely as African Americans to have weight loss surgery. On the surface, the data appear to signal racial disparity, but when researchers at Beth Israel Deaconess Medical Center dug deeper to ask why this variation exists, the answer was more complicated.

A new study of a 2008 expansion of Oregon's Medicaid program showed those with new coverage used emergency rooms 40 percent more often than other adults, throwing some water on the long-held argument that expanding Medicaid coverage lowers ER usage and boosts primary care visits.
