Avivia Health announces Avivia Select, a new series of affordable Disease Management and Wellness programs specifically designed to help companies provide important new benefits to their employees. Avivia Health is the only national provider of Disease Management and Wellness Solutions based on 60 years of evidence-based care by parent organization Kaiser Permanente.

Avivia Select is specifically designed for organizations with 5,000 to 25,000 employees and consists of three prepackaged solutions for individuals with chronic conditions and health risks. The programs are available nationwide and can be incorporated easily into virtually any health insurance program. All three Avivia Select solutions are based on a powerful Web 2.0 technology platform, which is a key advantage for employers who need varying levels of data reporting, analysis and information sharing at an affordable cost per employee.

Wellness conditions include tobacco cessation, weight management, and stress management. Chronic conditions include Asthma, Diabetes, CAD, COPD, CHF and associated conditions like Hypertension and Depression.

Level One includes a Health Risk Assessment (HRA) and a variety of interactive and educational tools and resources to support member self-care activities: access to Web-based modules and health information, web-based activity reporting, comprehensive employer communications tool kit, and optional incentives administration. Web content and tools are served up to the member based upon their web activities and health modules completed, including the HRA. Employer toolkits are offered to help employers create a company-wide culture of health and engage employees in a variety of activities.

Level Two includes all the capabilities of Level One, plus enhanced features such as unlimited 24/7 inbound calls to a health coach, and outbound Avivia interactive voice response (IVR) automated calls for those identified as high and moderate risk. Members can easily transfer to a live health coach from the automated calls. Risk identification is based upon self-reported HRA information and employee demographics. In addition, Level Two includes standard monthly Web-based reporting, which includes ROI calculations.

Level Three offers all the capabilities of the other solutions, but expands upon them with population segmentation, risk stratification and targeted outreach to moderate and high-risk members. Level Three features a combination of general awareness activities, inbound and outbound Health Coaching calls, Web-based campaigns, and Avivia interactive voice response (IVR) calls to reach and engage different populations. Level Three is a population-based program using HRA eligibility file, claims data algorithms for identification and risk stratification. Level Three includes standard monthly activity and outcome Web-based reporting, which includes ROI results.

Avivia Select is unique in that its member assessments, care plans, health coaching interventions, IVR services and data analytic algorithms are based upon Kaiser Permanente’s evidence-based clinical practices and research.
