Healthland, Jardogs partner to provide FollowMyHealth solution

Jardogs, a leading provider of patient engagement and health management solutions, has partnered with Healthland, the nation's largest provider of fully integrated solutions to rural community and critical access hospitals, to make their FollowMyHealth™ Universal Health Record solution available as Healthland's exclusive patient engagement portal. This collaboration will provide deep integration with Healthland's electronic health records (EHR) solutions, as well as help their 500-plus clients across the country meet Meaningful Use Stage 2 requirements.

FollowMyHealth™ (FMH) combines the global connectivity and patient management aspects of a personal health record with the integration of a patient portal to create the only true Universal Health Record.  FMH integrates directly into established workflows to provide a seamless, uninterrupted work process.  For patients, FMH provides a single point of access to their medical records, a secure online connection for interaction and communication with their providers, as well as a tool that enables them to take a proactive role in managing their own personal health and wellness.
