Chrononutrition shows promise for postpartum weight management

New research has found that eating habits that align most closely with a woman's existing routine show promise for losing postpartum baby weight, especially for mothers earlier in the postpartum period. The study identified key factors impacting eating timing and perceived barriers to various chrononutrition approaches, with the ability to share a family evening meal emerging as particularly important. The findings in the Journal of the Academy of Nutrition and Dietetics (JAND), published by Elsevier, are an important step towards informing the development of chrononutrition interventions for postpartum weight management.

Most mothers fail to return to their pre-pregnancy weight after childbirth. They frequently retain this weight, or gain additional weight, which contributes to an increased risk of becoming overweight or obese, worsening cardiometabolic health, and adverse outcomes in future pregnancies.

The postpartum period is an especially demanding life stage when it can be difficult for mothers to find time for traditional weight management approaches like keeping food records, counting calories, or attending weight loss counseling sessions. We wanted to examine alternative, potentially less burdensome weight management strategies that could promote mild calorie restriction, weight loss, and cardiometabolic health postpartum."

Time-based eating approaches that align eating with metabolic circadian rhythms-such as chrononutrition-have shown promise for weight management and cardiometabolic health and are growing in popularity. However, little research has focused on eating timing among postpartum women, including mothers' perceptions of how their eating timing has changed since having a baby and their thoughts on various chrononutrition approaches.

This study bridges that gap by investigating perceived determinants of postpartum eating habits and understanding the feasibility of following various chrononutrition approaches for weight management and cardiometabolic health from the perspective of postpartum mothers.

Through in-depth interviews and surveys with 10 mothers recruited from the University of Alabama at Birmingham Hospital within six months post-delivery, this research captured postpartum mothers' perspectives on their eating habits and various chrononutrition strategies. To triangulate the data, participants also shared their sleep schedules and ranked the discussed weight management approaches from most to least preferred.

Key findings

  • Weight-loss approaches that align with a mother's existing routine emerged as the most feasible. Infant schedules, availability of childcare, food preparation assistance and partners' work times were common factors influencing eating habits.
  • Sharing family mealtimes, particularly dinner as a central family activity, was an important factor when considering the feasibility of chrononutrition approaches postpartum.
  • Participants perceived the chrononutrition bedtime stopping rule approach (no energy intake within a certain number of hours before bedtime) and 10-hour time-restricted eating approach (shortening the daily eating window to a consistent 10-hour period) most favorably.
  • The "breakfast like a king, dinner like a pauper" approach (highest caloric intake at breakfast, lowest at dinner) was by far the least preferred approach and perceived as the least feasible among this group.

Dr. Schneider-Worthington comments, "I was surprised by how overwhelmingly disliked the 'king's breakfast, pauper's dinner' approach was. This was the only strategy that did not involve having specific eating start and/or stop times, but rather just redistributes the size of meals so breakfast is the largest and dinner is the smallest meal. This was also the only approach that raised concerns among breastfeeding mothers about how it would impact their milk supply. Lactating mothers may require education to help dispel the belief that large evening meals or overnight eating are necessary to support overnight milk production."

The investigators note that while this study is an important initial step in understanding the potential feasibility of various chrononutrition methods for postpartum weight management, it was limited by a small, relatively homogenous sample from the same geographical region. Future studies in larger, more diverse samples are needed to confirm these findings and further inform the development of postpartum-adapted chrononutrition interventions.

The investigators strongly recommend that new mothers interested in a chrononutrition approach should discuss it with their healthcare providers.

"For many, watching the clock is easier than counting calories or keeping food diaries. This simplicity could be especially helpful postpartum, when moms are often focused on caring for their infant and other children over themselves. Time-based eating could offer a straightforward, potentially achievable strategy to help them manage weight during an especially challenging time."
